Vital Statistic services provided by the Madison County Health Department:
- Provide vital statistics forms and instructions to those persons responsible for their completion.
- Maintain and file all Provisionals of death.A Provisional Report of Death serves as an initial notification of death that acts as a release for the body, notification that organs are to be donated, a record of who is responsible for filing the death certificate and/or the coroner’s authorization for cremation.
- Provide for voluntary acknowledgment of paternity services.
- Issue permits for disinterment and reinternment within the same cemetery.
