What is the Home and Community Based Waiver?

The Home and Community Based Waiver program is a caring connection to resources and services to help you or your loved one remain safe, secure, and stable at home.

An individual may receive the following services through HCBW:

  • Case Management
  • Attendant Care
  • Homemaking/personal care
  • Adult Day Health Care
  • Nutrition/meal delivery
  • Transportation
  • Goods & Services / Environmental Home Adaptation

Goods & Services / Environmental Home Adaptations

Items needed for personal care or to enhance the participant’s independence. For example: lift chairs, wheelchair ramps, Boost, Medical Alert system, etc.

Who may qualify for HCBW services:

Persons who are age 65 or older and/or have a physical disability.

Individuals who have a referral from their doctor

Those who are at risk for placement in a nursing home or long-term care facility.

Case Management

Our team of caring case managers will LISTEN to understand your priorities of service. They will create a service plan unique to each participant. Case managers coordinate services within the HCBW Medicaid funded program. The goal is to enhance a person’s level of independence and keep them safe in their home as long as possible.

The HCBW department at the Madison Co Health Department offers Case Management in the following counties: Boyle, Clark, Estill, Fayette, Garrard, Jackson, Jessamine, Laurel, Lincoln, Madison, Montgomery, Owsley, Powell, Rockcastle, Scott, Wolfe, and Woodford.

The Madison County Health Department HCBW also provides Attendant Care in Madison and Estill Counties

Professional attendant care aides are available to provide:

  • Light housekeeping
  • Personal care assistance
  • Meal preparation
  • Laundry
  • Medication reminders
