Open Records Requests

Requests for official files that do not contain PHI or confidential employee information are routinely received in Environmental Health for onsite and food establishment files.

• In most instances, these records are subject to the Kentucky Open Records Act.
• Individuals requesting to see or to have copies of these files will complete the following portions of Request to Inspect Public Records, Form OAG-01:
  • The requestor must enter their name, mailing address and email address.
  • The requestor must describe the specific records to be inspected.
  • The requestor must indicate their residency status.
  • The requestor must sign and date the request.
  • If the request is for copies and money is enclosed with the request, the requestor must indicate how much money is enclosed and the number of copies needed. There is a current charge of $.10 per copied page.
• The records custodian determines if the records are available and releasable.
• The records custodian will coordinate the release of anything that is non-routine with the branch supervisor and the Official Records Custodian.
• When copies are made and depending on the circumstances, certain personal information will be blocked with the use of a dark marker. Routine excluded information would include individual’s social security number, home addresses, and phone numbers.
